The Mid and North Coast HealthPathways Team have been celebrating reaching 400 localised HealthPathways!!
Mid and North Coast HealthPathways provides information to consistently assess and manage medical conditions as well as the criteria for requesting health services in the Mid and North Coast Local Health Area. HealthPathways empowers clinicians with locally agreed information to make the right decisions, together with patients, at the point of care.

SICK DAY ACTION PLANS
In conjunction with the 2018 Winter Strategy, Sick Day Action Plans have been updated on the HealthPathways website. The plans are available to download in pdf format and they are also available to upload into Best Practice and Medical Director Clinical software.
Sick day action plans are a great way to avoid unnecessary hospitalisation through improved patient self-management. Many of these action plans also have excellent detailed descriptions to support their use.
To access these plans, simply log into HealthPathways (username: manchealth – password: conn3ct3d) and click on the Blue Action Plans button icon on the homepage or type ‘Action Plans’ into the search function. The plans are also available via the North Coast Primary Health Network Winter Strategy webpage
NEW HOMEPAGE FOR HEALTHPATHWAYS!
HealthPathways new homepage has a refreshed and vibrant look! The new layout was created to align with new platform upgrades and to improve user experience.
Most of the old homepage content has been transferred over and some key information is now contained in Button icons.
We suggest you log on to HealthPathways and familiarise yourself to become acquainted with the new layout!
SUMMARY OF REFERRAL PAGES
This new look page provides quick access to local service providers and referral information. To access, simply click on the Service Provider Directory Button icon!
HealthPathways provides a “One Stop Shop’ when it comes to accessing all local service provider information and provides users with a “Step by Step” patient referral process.
The newly designed Summary of Referral Pages provides users with a comprehensive listing of Specialists, service providers and clinics available in our local footprint. For example, to view mental health services information, simply click on the Mental Health Referrals listing on this page.
