Thanks to the commitment of the Director of Integrated Care & Allied Health Services and the Tweed Director of Nursing & Midwifery, The Tweed Hospital will have a temporary Care Facilitator position appointed this Winter. The position reports to the Whole of Hospital Program Manager and will:
- Participate in Winter Strategy implementation and evaluation
- Drive and monitor Winter Strategy intiatives at the Tweed Hospital
- Facilitate communication between Hospital and Winter Strategy patients’ GPs and Practice staff
- Liaise with relevant staff in ED, inpatient wards, chronic disease management, Allied Health, community nursing, Hospital in the Home (HITH), and discharge planners to plan and provide safe transfer of care from hospital to home at the earliest opportunity
- mobilise resources to facilitate safe early discharge from hospital
Following a rigorous Expression of Interest process, Jaymie Hall was recruited in early June.
Jaymie was a Palliative Care Social Worker at The Tweed Hospital and Community Health Centre prior to being appointed as the Winter Strategy Care Facilitator for Integrated Care.
When asked what aspect of her current role she is enjoying most she said she was “enjoying liaising with different stakeholders who are all working towards the common goal of supporting people to manage their health care needs from the community”
What does Jaymie hope to achieve as the Winter Strategy Care Facilitator?

A bit more about Jaymie:
“I have been a Social Worker for 14 years working across the Emergency Department/ICU, Womens Care and Paediatrics Units and both the inpatient and outpatient Supportive and Palliative Care team. I am married with 2 children that keep us very busy leaving us wondering where our weekends go! If we aren’t at the beach for surfing with our son, we are at the netball courts with our daughter. Travelling is a must for our family with a new destination always on the horizon.”
