The 2018 Winter Strategy patient registration period has closed. The joint North Coast Primary Health Network (NCPHN) and Northern NSW Local Health District (NNSWLHD) initiative has entered the Service Delivery stage of the program.
Phase 3 runs from 28 May to 14 September. The key activities in Phase 3 are to:
- Deliver individualised care that is:
- Person-centered
- Comprehensive & Coordinated including WS18 Allied Health Funding, Admission and Discharge Notifications (fact sheet and troubleshooting guide available 28 May 2018) and Chronic Disease Management Team 48-hour post discharge follow up
- Has priority access for patients
- Use Quality Improvement processes to make change e.g. through PDSA cycles
- Participate in program evaluation learning and sharing
