NNSWLHD now requires a faxed referral coversheet for faxed referrals to all NNSWLHD Outpatient, Community and Allied Health Services.

The fax coversheet can be downloaded in 4 versions:

• Fax coversheet – to be filled on-screen – the LHD strongly prefers this version
• Fax coversheet – for printing and hand-filling
• Importable version for Best Practice – RTF inside a zip file. Please add to the front of your standard referral letter. Please do not change the field labels.
• Importable version for Medical Director – RTF inside a zip file. Please add to the front of your standard referral letter. Please do not change the field labels.
