The Northern NSW Integrated Care partnership (NNSWIC) has always been supported by NSW Health in trialling new ways of working to deliver care and addressing the needs of our community.
NSW Health is now working with NNSWIC and others across the state to scale up effective care initiatives making care more widely available.
Across NSW, 5 evidence based initiatives have been shortlisted.
All 5 proposed initiatives focus on improving outcomes for vulnerable, at risk populations and people with complex health and social needs in primary and community care where possible and appropriate.
The outcomes of the scaled initiatives will provide benefits for patients and staff, and contribute to NSW Health’s Quadruple Aim.
NNSWLHD, in collaboration with the integrated care partners are eager to implement the Integrated Care Vulnerable Families (ICVF) initiative.
ICVF is an initiative for families with complex needs.
Families may experience barriers to accessing health and social care, and are likely to have poorer health outcomes.
The initiative provides “whole-of-family” care in the community, supporting the unique needs of the individual patient and the family unit. This will be an exciting piece of work for the LHD.
