The NNSW Local Health District (LHD) has built an Admission and Discharge Notifications service (ADN) in conjunction with eHealth and in consultation with local GPs. ADNs have been built as a Proof of Concept (POC) as a statewide first to help improve patient outcomes and experience. What are ADNs? When a patient is enrolled in
