North Coast Primary Health Network is excited to announce the establishment of an innovative quality improvement initiative based on the Patient Centred Medical Home model. The project aims to strategically position our region as a key contributor to the development of the Patient Centred Medical Home model in Australia.
What is a Patient Centred Medical Home (PCMH)?
The Patient Centred Medical Home, referred to by some as a Health Care Home, is a model for general practice, chosen by a person to be responsible for the coordination of their ongoing, comprehensive and whole of person medical care. A Patient Centred Medical Home coordinates care delivered by all members of a person’s care team. Internationally, the Patient Centred Medical Home has demonstrated a successful model for providing effective, efficient and economical outcomes for both practitioners and patients. What is the focus of the PCMH project?
- Producing the Australian Handbook for transitioning to a Patient Centred Medical Home
- Establishing a local network of practitioners and practices focused on implementing Patient Centred Medical Homes
- Conducting a local trial and adaptation of the handbook tools
