Despite a challenging year 2021 saw the Integrated Aboriginal Chronic Care (IACC) partnership continue to provide an integrated approach to care for Aboriginal persons living with chronic illness across all Aboriginal specific and mainstream chronic care programs and services throughout NNSWLHD.
Vimbayi “Vee” Mupudzi, Integrated Aboriginal Chronic Care Coordinator, NNSWLHD has continued to co-locate within Aboriginal Medical Services (AMSs) across the NNSWLHD footprint. (Grafton, Casino, Lismore, Ballina and Tweed Heads) when COVID restrictions permitted of course.
The “No Wrong Door” referral system is the lynch pin of the IACC program which has seen improved outcomes for Aboriginal Chronic Care patients in Northern NSW. In true partnership IACC has capacity to empower its clients by ensuring the right care, in the right place at the right time is continually provided and this is the fundamental element of Integrated Care.

Vimbayi is looking forward to the new year and continuing to collaborate with inpatient medical and nursing staff regarding chronic care services that are specific for Aboriginal people and continue to co-locate within Aboriginal Medical Services.
