The Ministry of Health (MOH) visited Ballina on Wednesday 26 September to discuss the statewide Integrated Care Strategy and the Northern NSW Integrated Care partners provided them with an update on the local strategy.
MOH delegates included Brad Astill, Executive Director, System Performance Support, Marisa Rose, Director, Integrated Care, Sarah Sherborne-Higgins, Manager, Integrated Care Implementation and Robyn Grant, Technical Writer, System Performance Support Branch.
The agenda for the day included, new models of care, systems integration, shared care plans and risk stratification.
Following the 4 hour update, Marisa Rose and Robyn Grant provided staff including Chronic Disease Management (CDM) staff and managers a demonstration of the soon to new Integrated Care Patient Flow Portal module.
