How does NNSW Integrated Care partnership work?

Northern NSW Integrated Care recognises the need for a strong local partnership in order to ensure success of this initiative.

The partners involved are commmited to the agreed purpose and objective. This is the delivery of holistic patient care that responds to all of a person’s health needs, across physical and mental health, in partnership with the patient, their carers and family.

A great importance is placed on trust as well as clear partnership arrangements. Along with good communication between partners this will ensure that integrated care will become common practice in local health care services of the Far North Coast of NSW.


Who Are Our Partners?

The Northern NSW Integrated Care initiative is a collaborative effort between the Northern NSW Local Health District, North Coast Primary Health Network, Aboriginal Medical Service, NSW Ambulance and local General Practices.
