SESSION 1: TRANSFORMING YOUR PRACTICE: PROGRESSING YOUR MEDICAL HOME
Monday 6 November 6pm – 9pm
This session will explore the successes and challenges of the Patient Centred Medical Home model in the USA and will look at learnings for the uptake of the PCMH model in Australia.
SESSION 2: ENGAGING AND EMPOWERING PEOPLE AND COMMUNITIES
Thursday 9 November 1pm – 4.30pm
This session will outline the rationale and evidence that lies behind the need to engage and empower people and communities within integrated care systems as a means to achieving better health outcomes. By drawing on international experiences, the session will examine the range of tools and practical approaches that can be used to enable people to live well.
SESSION 3: HEALTHY TOWNS AND PLACE BASED CARE
Thursday 9 November 5:45pm – 8:45pm
This session seeks to understand how to support healthier communities through new, multipartner, approaches to care with a focus on preventative and place based population health. It will draw upon international experiences to examine the Healthy Town movement and methods/approaches that have been used to develop place-based care that maximises the assets within a local community to support health and wellbeing.
SESSION 4: FRAILTY, INTERMEDIATE CARE AND COMPLEX CONDITIONS
Friday 10 November 9am – 12.30pm
This session will examine international best practice in the management of people with complex care needs in community settings, with a focus on approaches to intermediate and transitional care including step-up, step-down models, and to people with severe and complex mental health conditions. A specific focus will be on how to deal with seasonal surges and winter pressures.
SESSION 5: RE-ORIENTING THE MODEL OF CARE
Friday 10 November 2pm – 5.30pm
This session will provide an understanding of international trends in the re-orientation of care models to promote integrated care and population health. It will examine international and domestic strategies in the creation of patient-centred medical homes and accountable care systems drawing on case examples and learning from the USA and Europe.
