Update GP or Practice Details with LHD
Use this form to update the information the LHD holds for your GPs and Practice in our hospital system address books.
It is crucial we have the correct GP names to ensure that Discharge Summaries and Admission/Discharge Notifications (ADNs) are delivered correctly.
If you would like an easier way to update the LHD, Secure Message Brokers (HealthLink and Argus), the National Health Services Directory and other services about your practice and the GPs at it, then consider using the Australian Digitial Health Agency’s Service Registration Assistant.
