SOS clinic operational from 29th May 17

(SOS clinic screening for whole of health issues effecting joint pain and function. Assessment and management program put in place, GP recommendations and reporting. Templates for referral-sourceassessment developed)

      1. 45 operational days since opened
      2. 93 referrals
      3. 98 OOS (64 Initial, 33 x 1 month follow up, 1 x 3month follow up)
    2. Sources of referral
      1. 50% via specialists, 44% via GP’s
      2. 4 Internal referrals (Tweed Physio)

      2. Over 325 communications have been sent to GP’s or Specialist from the SOS clinic via Medical Objects
    2. Referral Acknowledgement – On Track

      1. 100% completed within 4 weeks of referral (goal 100%)

6-week group exercise and education program to kick-start exercise and lifestyle change. Currently running Weds only. Commenced 30th June.

  1. 15 x myOA group exercise sessions (4-7 attending each session)
  2. Presenters engaged for education sessions
  3. 9 patients have completed program
  4. PRM questionnaire developed and tablet now available
