eReferrals Help Request Page
Please contact HealthLink in the first instance (details below) if you (as a referrer) are experiencing issues with HealthLink Smartforms including:
- You cannot find HealthLink Forms in your practice management/clinical information system (and it is one of the supported systems).
- You can find HealthLink Forms but they do not load or there is an error on loading
- eReferrals are not being sent (usually this is accompanied by an error message with the form being parked for sending later)
Contact HealthLink on 1800 125 036 or on helpdesk@healthlink.net if there are issues with. For escalations, contact the LHD support team using the below form.
If you can load HealthLink forms and see Transport NSW and/or Commonwealth My Aged Care but can’t see NNSWLHD eReferrals please contact the LHD project team using the form below.
